My $.02...

I think it's unnecessary to have two forms.  Just have the workgroup selection take place on a previous page which directs you to the workgroup specific join page.  Then the exact same form is used no matter how you go there.

Comments on the WG/DG join screen:

*         There should be a link to the Workgroup charter (probably the workgroup name in section (4)).

*         5 should be more specific as to the IPR policy for the workgroup rather than how it is currently worded fairly generically.  Perhaps something like "I hereby agree to the Creative Commons IPR Policy (Kantara IPR Option 3.7) as designated in the "User Driven Volunteered Personal Information Tech" Workgroup Charter."

*         I think that (6) is redundant and unnecessary if you are going to have the company representative vet each join anyway.   The representative will be the one who can do that.   I certainly don't have the authority to execute an agreement on behalf of Intel and wouldn't be able to meet this condition.

*         I think the checkboxes on both (5) and (6) are unnecessary.

*         The required fields should have flags to indicate that they are required (or else a statement at the top that all fields are required).

*         It would be nice if I didn't have to re-do the captcha if I made a mistake.

Sure thing Trent, the more review the better (hopefully).  We have these online now to "demo".  But no one should use these today under the impression that its legally binding.  We will flush ever request we see come in until the BoT approves the process as binding to our IPR Policy.  With that said:
