[KI-LC] Group "Join" Form

I'm about to review this information, but wanted to let you know that
it's now 9am EDT on Monday when I'm getting to this (i.e. T-1 hour for
comments).  Due to the short time available for review, it's likely that
we'll collectively miss something and need to revisit this.
