[KI-LC] Group "Join" Form

First of all, I'm not sure who created the HTML for this form, but it's
relatively old school. It relies heavily on tables rather than cascading
style sheets, and as such will be less flexible. I'd suggest having
someone bring it up to XHTML standards.

That being said, I've attached is a modified version of the form (in the
same vein) that illustrate my suggested changes:

1. Improved information architecture for readability.
2. Broken out the "Company" and "Affiliation" entries
into separate lines to clarify what's being asked.
3. Changed the WG/DG options into a selection box to
allow for multiple selections in one pass.
4. Added headings to the IPR and Authority sections
to simplify readibility.
5. Added introductory text clarifying the process of
submission for the form.
6. Tweaked some of the text for comprehension.

Beyond that, I'd suggest the following additions to the form:

A. Include client-side validation (and matching server-side) for:
i. All fields being filled out (with exception B)
ii. Email address validation of: syntax and MX validity
B. Client-side validation should allow for the "Company"
field being left blank if (4) is set to "Individual"
C. Remember all filled/selected items upon validation failure
D. Add clarifying text to the receipt page with information
on how / when their application will be accepted.

I know I'm already late with my response, and I'm sure I've missed
something (and could have suggested better solutions). I hope this helps
us move forward, though.
